Live in and transition support

At Uncovery, we recognise how challenging it can be to readjust to your home environment after receiving in-care treatment for disordered eating. That's why we offer a personalised live-in support service designed to cater to your specific needs and provide support when and where you need it the most. Our transition assistance is aimed to help make the transition to independent living smoother, with the right level of emotional and practical support.

How we'll work together

We want to help you to build the skills you need to navigate recovery back at home following in-care treatment. Being with you in real time can help us be with you to implement these new skills with confidence and support.

As a first step, we will work with your treatment teams and do an initial assessment with you to get to know you and identify and understand your unique needs. This helps us create an individualised plan that is suitable to those needs and in line with your goals.

From there, we begin implementing these plans in collaboration with your treatment teams. One of our coaches can stay with you at your home during this time, or you can opt for you both to stay elsewhere (such as a hotel).

Some of the elements we can build into your individualised plan:

  • Setting up your home to support your recovery (removing triggers such as scales, old photographs, clothing that no longer fits, etc.)

  • Meal Support, including the preparation of meals and snacks in the home as well as creating more variety in food choices (in accordance with your meal plan provided by your dietitian)

  • Restaurant Outings: learning how to navigate a menu and the restaurant environment

  • Identifying and challenging food rules and fears

  • Building effective strategies for during and after meals

  • Changing behaviours and rituals around food

  • Planning and executing grocery shopping

  • Exploring your ambivalence and working with your level of motivation and stage of change

  • Creating a healthy relationship with exercise. (e.g., attending a yoga or exercise class as appropriate with guidance from your treatment team)

  • Attending medical/health/treatment appointments with you

  • Helping you to challenge/manage triggers/behaviours and urges if/when they arise

  • Clothes shopping to fit your stage of recovery

  • Helping you to build and practice strategies to deal with/manage uncomfortable thoughts and feelings

  • Creating a sustainable support network to help you once we leave. We aim to help you to build up your life as we step down our support.
